Book a Flight Physical

As a flight surgeon with the 310th AMDF Reserves, Dr. Langley is excited to be newly training to perform civilian aeromedical exams. For the first two years in practice, she is restricted to Class II and III exams.


Common conditions which require additional paperwork:

Arthritis

You must submit a current status report from your treating physician and any supporting documents to demonstrate you meet the following criteria:
* Stable condition on current regimen
* Symptoms - none to mild/moderate with no significant limitations
* Causes are limited to rheumatoid, psoriatic or osteoarthritis
*Acceptable medications of oral steroids not exceeding equivalent of 20 mg/day, methotrexate, NSAIDS, or Plaquenil (requires mandatory eye exam-submit completed FAA Form 8500-7))
*Labwork of CBC and complete metabolic panel within the last 90 days

Asthma

You must submit a current status report from your treating physician and any supporting documents to demonstrate you meet the following criteria:
*Stable condition on current regimen
*Symptoms are stable and well-controlled (either on or off medication):
*Use of oral corticosteroids for exacerbations no more than two times per year
*No inpatient hospitalizations within the last year
*Use of acceptable medications of inhaled short/long acting beta agonist (e.g. albuterol), inhaled corticosteroid or leukotriene receptor antagonist (e.g. Singular)
*Pulmonary Function Test (PFT) is required if using daily medications

Bladder Cancer

You must submit a current status report from your treating physician and any supporting documents to demonstrate you meet the following criteria:
*Stable condition - (no recurrence or spread outside the bladder)
*Active treatment is completed (chemo/radiation)
* Surgery - off pain medication, fully recovered and released by surgeon
Note: If it has been 5 years or more since any treatment for this condition, with no history of metastatic disease and no reoccurrence, no documentation is needed.

Chronic Kidney Disease

You must submit a current status report from your treating physician and any supporting documents to demonstrate you meet the following criteria:
*Stable condition
No new conditions or complications affecting real function
Any underlying conditions (such as diabetes, HTN, glomerulonephritis, PKD or chronic obstruction)
Comments that dialysis or transplant are not recommended or anticipated at this time
*Documentation that GFR is 35 or higher, urine albumin is trace or negative and hemoglobin is at least 10 gm/dL/hematocrit at least 30% (labs within the last 6 months)
*Current treatment

Colitis

You must submit a current status report from your treating physician and any supporting documents to demonstrate you meet the following criteria:
*General health status due to this condition
*Symptoms (diarrhea with/without mild abdominal pain and cramping; fatigue)
*Cause of colitis (Crohn’s Disease, Ulcerative Colitis, or Irritable Bowel Syndrome)
*Surgery within the last 6 weeks (Yes/No)
*Acceptable Medications- oral steroids not exceeding equivalent of prednisone 20 mg/day
Imran or Sulfasalazine
Mesalamine (Asacol, Pentasa, Lialda, etc.)
Steroid foams or enemas
Loperamide less than or equal to 16 mg/day with no side effects
Hyoscyamine-use 1-2 times/week with no side effects and no fly 48 hrs after use

Glaucoma

You must submit a current status report, and a completed FAA Form 8500-14, from your treating ophthalmologist and any supporting documents to demonstrate you meet the following criteria/addresses the following:
*Stable condition on current regimen
*Age at diagnosis
*Glaucoma type
*Documented nerve damage or trabeculectomy
*Medications and side-effects, if any
*Intraocular pressure of 23 mm Hg or less in both eyes
* Any evidence of defect in visual fields

Hepatitis C

You must submit a current status report from your treating physician and any supporting documents to demonstrate you meet the following criteria:
*Stable condition on current regimen
*Complications or symptoms from Chronic Hepatitis C
*Labs - include liver function tests, albumin and PT (within last 90 days)

Hypertension

You must submit a current status report from your treating physician and any supporting documents to demonstrate you meet the following criteria:
*Stable condition on current regimen for at least 7 days (no changes recommended and medications are well tolerated)
*Symptoms
*Medications

Hypothyroidism

You must submit a current status report from your treating physician and any supporting documents to demonstrate you meet the following criteria:
*Stable condition on current regimen
*Symptoms
*Medications
*Labs - TSH within the last year

Kidney Stone(s) (Nephrolitiasis, Renal Calculi or Renal Colic)

You must submit a current status report from your treating physician and any supporting documents to demonstrate you meet the following criteria:
*Stable condition - no increase in number or size of stones
*Asymptomatic - unlikely to cause an incapacitating event
*Surgery- if performed, patient is fully recovered, off pain medications with release from surgeon
*History of complications
*Underlying cause for stone recurrence?
*Current or recommended treatment

Migraine and Chronic Headache

You must submit a current status report from your treating physician and any supporting documents to demonstrate you meet the following criteria:
*Stable Condition on current regimen
*Acceptable Type of Migraine Headache (Classic/Common Migraine, Chronic Tension headache, Cluster headache)
*Frequency
*Symptoms
*Medications - preventive and abortive

Mitral Valve Repair

You must submit a current status report from your treating physician (cardiologist) and any supporting documents to demonstrate you meet the following criteria:
*Surgery for mitral valve repair was 5 or more years ago
*Stable condition (asymptomatic) with no other cardiac conditions
*History of any connective tissue disorder, lung disease, pulmonary hypertension or other cardiac disease
*ECHO results (performed within the last 24 months)

Pre-Diabetes

You must submit a current status report from your treating physician and any supporting documents to demonstrate you meet the following criteria:

* Stable condition on current regimen
* Symptoms
Hypoglycemic events (symptoms or glucose least than 70 mg/dL within the past 12 months)
* Medications for condition
*Labs: fasting blood sugar, current Hemoglobin A1c (within last 90 days)

Prostate Cancer

You must submit a current status report from your treating physician and any supporting documents to demonstrate you meet the following criteria:
*Stable condition with no spread or recurrence
*No evidence of metastatic disease
*Active treatment completed (chemo/radiation) with no further treatment currently recommended
*Surgery-if performed, patient is fully recovered, off pain medications with release by surgeon
*Current PSA (within the last 6 months)
*Any current treatment
Note: If it has been 5 or more years since any treatment for this condition, with no history of metastatic disease and no reoccurrence, no documentation is required.

Renal Cancer

You must submit a current status report from your treating physician and any supporting documents to demonstrate you meet the following criteria:
*Stable condition on current regimen
*Evidence of disease recurrence, extra capsular extension, metastatic disease, Stage 4 disease or Paraneoplastic syndrome
*Treatment completed and back to full unrestricted activities
Note: If is has been 5 or more years since any treatment for this condition, with no history of metastatic disease and no reoccurrence, no documentation is required.

Testicular Cancer

You must submit a current status report from your treating physician and any supporting documents to demonstrate you meet the following criteria:
*Stable condition with no spread or recurrence
*No evidence of metastatic disease
*Active treatment is completed (chemo/radiation) with no further treatment pending
*Surgery-if performed, patient is fully recovered, off pain medications with release by surgeon
*Symptoms
*Any current treatment

Note: If it has been 5 or more years since any treatment for this condition, with no history of metastatic disease and no reoccurrence, no documentation is required.
